Cut no-shows with a simple reminder routine

Tristate Health4 min read

Most no-shows are not patients who stopped caring. They are patients who forgot, could not get through to reschedule, or did not realise they needed to. A consistent reminder routine fixes most of that.

The cadence

Two reminders, sometimes three.

At booking. Confirm the appointment then and there, by text or email, while the patient is still engaged. This also catches wrong contact details immediately rather than a week later.

Two to three days before. This is the one that does the work. It is far enough ahead that the patient can still reschedule into a slot you can refill, and close enough that it registers.

The morning of. A short nudge. Useful for busy clinics, optional for others.

A reminder one hour before an appointment does not reduce no-shows. It just tells you about one sooner.

What the message says

Keep it short and keep health details out of it. Practice name, date, time, location, and how to confirm or reschedule.

Hi Sam, this is a reminder of your appointment at Tristate Family Medicine on Tue 14 Oct at 2:15pm. Reply C to confirm or call (718) 689-3087 to reschedule.

Do not include the reason for the visit, the provider's specialty where it implies a condition, or anything about treatment. If your practice name itself discloses sensitive information, that is worth a conversation before you set up any automated messaging.

Use a platform that will sign a business associate agreement, and record each patient's contact preference.

Make rescheduling genuinely easy

This is the half most practices skip. A reminder that a patient cannot act on is just a notification.

Give them a way to reschedule that does not involve waiting on hold: a reply, a link, or a number that a person actually answers. A patient who reschedules is not a no-show, they are a kept appointment on a different day, and you get the slot back with enough notice to fill it.

Track it, then adjust

Count no-shows by provider, by day of week, and by appointment type. The pattern is usually specific rather than general: a particular slot, a particular visit type, new patients rather than established ones.

Once you can see the pattern, you can act on it. Maybe new patients need a phone call rather than a text. Maybe Monday mornings need a confirmation on Friday. You cannot find that out without counting.

Before you add a policy

Practices often reach for a no-show fee first. Consider it after the reminder routine is running properly, not before. Fees are hard to collect, awkward at the front desk, and they do nothing for the patient who simply forgot.
