Privacy policy
How we handle information submitted through this website, who we share it with, and the choices you have.
Last updated November 2026
What this policy covers
This policy covers tristatemso.com and the forms on it. It explains what we collect from visitors to this website and what we do with it.
It does not cover protected health information held by a medical practice we work with. That information is governed by that practice's own notice of privacy practices, and by the agreement between us and that practice. See our HIPAA notice for how we handle information on behalf of client practices.
Information we collect
We collect only what you choose to send us:
- Contact form: your name, practice name, email address, phone number if you provide one, specialty, number of providers, the services you are interested in, your message, and your preferred contact method.
- Newsletter: your email address.
- Automatically: aggregate, privacy-friendly analytics about page visits. We do not use advertising cookies or cross-site tracking, and we do not build profiles of individual visitors.
Please do not send patient information
This website is not a secure channel for protected health information. Do not include patient names, medical details, or any other patient information in a website form or in an email to us. If you are a client and need to send us PHI, use the secure channel set out in your services agreement.
How we use it
We use what you send us to:
- Reply to your enquiry and discuss whether we can help your practice.
- Send the newsletter you asked for, until you unsubscribe.
- Understand which pages are useful, in aggregate, so we can improve the site.
- Meet our legal and record-keeping obligations.
We do not sell your information
We do not sell personal information, and we do not share it for cross-context behavioural advertising. We do not send your information to advertising networks or data brokers.
Service providers
We use a small number of providers to run this site and handle messages. Each receives only what it needs to do its job, and each is bound by its own agreement with us:
- Vercel: website hosting and privacy-friendly, cookieless analytics.
- Resend: delivery of contact form notifications, confirmations and the newsletter.
- Cal.com: scheduling, if you book a call. What you enter on the booking page is handled under Cal.com's own privacy policy.
- Cloudflare: bot protection on our forms, where enabled.
Cookies and tracking
We do not use advertising or cross-site tracking cookies, so this site does not show a cookie consent banner. Our analytics are aggregate and cookieless. Essential storage may be used to make the site work, such as remembering that you dismissed something.
If you book a call, the scheduling provider may set its own cookies on its booking page. That is governed by its privacy policy, not this one.
How long we keep it
We keep enquiries for as long as needed to respond and to maintain a record of our business communications, and then delete or archive them. Newsletter subscriptions are kept until you unsubscribe. You can ask us to delete your information sooner at any time.
Security
The site is served over HTTPS. Form submissions are validated and rate limited, and we do not log the contents of what you send. Access to enquiries is limited to the people at Tristate Health who need it. No method of transmission over the internet is completely secure, which is why we ask you not to send patient information through this site.
Your choices and rights
You can unsubscribe from the newsletter using the link in any email we send. You can ask us what information we hold about you, ask us to correct it, or ask us to delete it.
Depending on where you live, you may have specific rights under laws such as the California Consumer Privacy Act or the GDPR, including the right to access, correct, delete, or receive a copy of your information, and the right not to be discriminated against for exercising those rights. We honour these requests regardless of where you live.
To make a request, email info@tristatemso.com or call (718) 689-3087. We may need to verify your identity before we act, and we will respond within the time the applicable law allows.
Children
This site is intended for medical practice owners and administrators. It is not directed to children, and we do not knowingly collect information from anyone under 16.
Changes to this policy
If we change this policy we will update the date at the top of this page. Significant changes will be described here rather than made quietly.
