CCM & RPM questions
More care management answers →What is Chronic Care Management (CCM), and can my practice bill for it?
CCM is ongoing care coordination between visits for patients with two or more chronic conditions. With the patient's consent and a care plan in place, Medicare pays monthly for the clinical staff time spent managing their care.
Which patients qualify for CCM?
Patients with two or more chronic conditions expected to last at least 12 months that put them at risk, such as diabetes, high blood pressure, COPD or heart failure. They must agree to take part.
What is Remote Patient Monitoring (RPM)?
RPM uses connected devices, like blood pressure cuffs or glucose meters, that send readings to your practice automatically. Your team reviews the data and steps in when something changes.
Do I need to hire staff to run a care management program?
No. Our care managers handle enrollment, check-ins, documentation and time tracking under your practice's supervision. Your providers stay in charge of clinical decisions.
